Deadly Choices birth certificate initiative

Hey new parents don’t forget to register your bub’s birth.

Commemorate their birth with a Deadly Choices commemorative certificate.

A birth certificate gives your bub access to their rights, we are working with:

  • Institute for Urban Indigenous Health (IUIH), and
  • Townsville Aboriginal and Islander Health Services (TAIHS).

Who can apply

Parents can get a Deadly Choices birth certificate package (including a standard birth certificate), if:

  • their child is 5 or younger
  • they are patients at a participating IUIH or TAIHS clinic, and
  • they are up to date with their vaccinations and health checks.

How to apply

To apply for your child's free birth certificate package, you need to:

  1. Get a Deadly Choices birth certificate package application from reception at your clinic.
  2. Complete the application form.
  3. Submit your application form in person, by email or by post.

This application form is only available from participating clinics and is not available from us.

Return the completed application form to the staff at your clinic.
