Victim support agencies can refer victims to the QCS Victims Register.
Who can use this service
You can refer a victim if:
- you work for a victim support agency
- you are a victim support officer
Don't use this service if:
- you are a victim of crime
Instead, you can register as a victim of an adult offender.
How to refer a victim to the QCS Victims Register
Choose how to refer and follow the steps.
Step 1: Gather information and documents
Gather information about the victim:
- name
- date of birth
- contact number
- offender details from the crime report.
Step 2: Proof of identity
Attach 1 copy of the victim’s proof of identity, either:
- driver licence
- passport
- photo ID.
Step 3: Complete the form
Download, print and complete the application form.
Step 4: Victim declaration
The victim you are referring must sign the declaration.
Step 5: Submit the application
Send the complete application and supporting documents to:

Step 6: Receive the outcome
You will receive confirmation the referral has been received.
