Proof of identity documents for victims of crime

Applicants making a claim for financial assistance for victims of crime need to attach clear, readable photocopies, scanned copies or photos of their proof of identity, from either:

  • 1 list (A) document
  • 2 list (B) documents.

If you are using an online application form, you can upload photos or scanned copies of your documents. Photos are the easiest to upload. Check the list below to find out what documents can be uploaded.

If you are posting your application, you only need to post photocopies of your documents. Do not post original documents with your application. Check the list below to find out what documents may be needed.

List A

Attach 1 of these:

  • Australian birth certificate (full) or overseas equivalent
  • Australian citizenship certificate or naturalisation certificate
  • Australian driver licence (current or expired less than 2 years) or overseas equivalent, either:
    • a photo or copy of the physical licence
    • an image of the digital licence showing all information (a screenshot is acceptable).
    • we cannot accept a digital licence that is shared via the Queensland Digital Licence app.
  • Australian firearms licence
    • a photo or copy of the original (first) licence and a photocopy of the current licence
  • Australian or foreign passport (current or expired less than 2 years)
    • a photo or copy of the page with your photo on it
  • Commonwealth or state public service ID card with photo and signature (current, not expired)
  • Queensland photo identification card (formerly called proof of age card) or interstate/overseas equivalent, either:
    • a photo or copy of the photo identification card
    • an image of the digital photo identification card (a screenshot is acceptable)
    • we cannot accept a digital photo identification card that is shared via the Queensland Digital Licence app
  • record of immigration status (certificate of evidence of resident status)
  • travel documents and current Australian visa.

List B

Attach 2 of these:

  • Australian exam report (for people aged under 16 years old only)
  • Australian record of achievement (for people aged under 16 years old only)
  • Australian secondary school exam certificate (for people aged under 16 years old only)
  • Australian Medicare card
  • marriage certificate issued by a Registry of Births, Deaths and Marriages
  • bank account card (EFTPOS card) or credit card with embossed name
    • a copy of the front of the card only
  • bank passbook, or bank statement with name
  • Centrelink or Department of Veterans Affairs card
  • change of name certificate
  • council rates notice, rental agreement, or utility bill (with name and address)
  • letter from employer (current or within last 2 years)
  • secondary school photo ID card (less than one year old and issued by an Australian secondary school)
  • security guard/crowd control licence (Australian issued)
  • tertiary ID card (less than one year old and issued by an Australian university).

If the application is for a child (under 18)

If a parent, guardian or other pre-approved adult is completing the form for a child, they must:

  • attach clear and readable photocopies, scanned copies or photos of
    • 1 list (A) document or 2 list (B) documents for the parent, guardian or pre-approved adult (not the child).

If a child is over 12, living independently and being helped by a lawyer, they must:

  • attach clear and readable photocopies, scanned copies or photos of
    • 1 list (A) document or 2 list (B) documents for the child.

If a government worker is making an application for a child under the care of Child Safety Services, the worker must:

  • attach clear and readable photocopies, scanned copies or photos of both
    • a document confirming they are the legally appointed guardian of the child
    • 1 list (A) document or 2 list (B) documents for the child.

If the application is for a person who has impaired capacity

If a guardian, administrator, attorney or another pre-approved adult is completing the form on behalf of a victim who has impaired capacity, they must:

  • attach clear and readable photocopies, scanned copies or photos of all of these documents
    • 1 list (A) document or 2 list (B) documents for the guardian, administrator, attorney or pre-approved adult
    • a document confirming they are a legally appointed guardian, administrator, attorney (if applicable)
    • 1 list (A) document or 2 list (B) documents for the victim.
