You can end an advance health directive at any time, if you have capacity to make decisions.

If you lose capacity, you cannot revoke your advance health directive.

Who can use this service

You can end an advance health directive if you:

  • are 18 years or older
  • have the capacity to understand the decision and what it means for you.

Don't use this service if:

  • you are under 18
    Instead, speak to your parent or guardian about your health care preferences.
  • you don’t have capacity to understand the decision and what it means for you
    Instead, your attorney will continue to make decisions for you.

How to end an advance health directive

Follow these steps to end an advance health directive.

Step 1: Write a statement

Write a statement that you are revoking your advance health directive. There is no form for this.

Include your full name, date of birth, and the date the advance health directive is revoked.

Step 2: Notify relevant parties

Tell anyone who has a copy of your advance health directive that it's revoked.

This may include your:

  • attorney(s), if appointed
  • family or close friends.
  • local hospital (if they have a copy on file)
  • lawyer or legal representative
  • doctor or other health care providers

Step 2: Give a copy to relevant parties

Provide a copy of your written statement to the relevant parties listed above.

If you have the capacity to make decisions: immediate
