Industry authority remote area kit

Queensland residents who live in remote areas and want to apply for an industry licence or driver authorisation are able to lodge their applications using a remote area kit.

A remote area kit may be used when applying, renewing, or changing/replacing the Queensland Industry Authority for industry licences and driver authorisations.

Industry licences include:

tow truck driver and assistant
traffic controller accreditation
driver & rider trainer accreditation
dangerous goods driver
pilot/escort driver.

Driver authorisations include:

booked hire/taxi (BHTX)
general (GENR).

Authority to check a visa holder’s entitlement form (F4595) (PDF, 67KB)—this is required if you are applying for a driver authorisation and require a visa entitlement verification check.
A licence specimen signature form (F2127) (PDF, 55KB)—this may be required with your application.
A medical certificate for motor vehicle driver form (F3712) (PDF, 111KB) and private and commercial vehicle driver's health assessment (form F3195) (PDF, 82KB)—these forms are required for driver authorisation and dangerous goods applicants.
Evidence of identity requirements information sheet (S5385) (PDF, 103KB)—this form contains a list of required identity documents.
