If you are unable to download these forms, you can contact your local patient travel office and ask them to email or post you a copy. Alternatively, you can get a copy of these forms from your local hospital or health facility. Check the contact details for your local health facility or patient travel office.
Patient registration—(Form A)
Use this form to register with the Patient Travel Subsidy Scheme (PTSS). If you are already registered with the PTSS and your details have not changed you do not need to re-register. If your details have changed since you registered, use this form to update them.

Travel application—(Form B)
This form is completed by the patient's GP or specialist. It needs to be submitted as part of the patient's application for a PTSS travel subsidy.

Appointment attendance—(Form C)
Use this form to confirm that a patient has attended their specialist appointment. This form needs to be submitted in order for a patient to claim their travel subsidy payment. A new form is needed for each appointment.

Accommodation attendance—(Form D)
Use this form to confirm that a patient has attended their approved accommodation. This form needs to be submitted in order for a patient to claim their travel subsidy payment.

Repatriation request—(Form E)
This form is to be completed by the administering Hospital and Health Service (HHS) to support the repatriation of a deceased patient.

Patient Vendor Banking Details
Use this form to register your bank account details with the Patient Travel Subsidy Scheme (PTSS). If you are already registered with the PTSS and your details have not changed you do not need to re-register your banking details. If your banking details have changed since you registered, use this form to update them.
