Replacement acknowledgement of sex birth certificate
If you have already altered your record of sex with us, you can apply for a replacement Queensland linking birth certificate showing your current record of sex.
When the sex that you identify as has not already been registered, you must apply to register it.
If you were born interstate or overseas, you need to apply for a replacement recognised details certificate.
How to apply
- Check that you are eligible to apply.
- Download, print and fill in the replacement acknowledgement of sex certificate application form.
- Have your supporting documents certified by a Justice of the Peace or another authorised person (if applying by post).
- Attach your certified proof of ID documents and additional supporting documents (as required).
- Submit your signed application form, along with payment and supporting documents by post or in person.
