Replacement recognised details certificate

If you have already had your sex recognised with us and were born outside of Queensland, you can apply for a replacement Queensland recognised details certificate.

You can choose an official:

  • linking recognised details certificate—shows current and previous sex details
  • recognised details certificate—shows only current details.

If you changed your name when you had your sex recognised, you can also order an official change of name certificate. This certificate will show your current and previous names.

If the sex that you identify as has not already been recognised in Queensland, you need to apply to have it recognised.

If you were born or adopted in Queensland, you need to apply for a linking birth certificate.

How to apply

  1. Check that you are eligible to apply.
  2. Download, print and fill in the replacement recognised details certificate application form.
  3. Have your supporting documents certified by a Justice of the Peace or another authorised person (if applying by post).
  4. Attach your certified proof of ID documents and additional supporting documents (as required).
  5. Submit your signed application form, along with payment and supporting documents by post or in person.
