Replace your tattoo licence
If your licence is lost, stolen, destroyed or damaged, you must apply for a replacement licence or certificate.
You will need to contact us in writing. You must include your:
- name
- address
- type of licence
- licence number (if you know it)
- reason for a replacement.
You must pay a licence replacement fee.
