Driving when you're 75 and over

Prepare for your doctor's appointment

  1. Make an appointment to see your doctor. Let them know that you would like your fitness to drive assessed as they may need to allow more time than a standard visit.
  2. Complete part 1 of the Medical Certificate for Drivers (F3712) form before your appointment to give to your doctor.
  3. Complete part 1 of the Private and Commercial Vehicle Driver’s Health Assessment (F3195) form, if possible, as this will help your doctor.
  4. Make sure you have your driver licence with you.
  5. Take your glasses or contact lenses with you, if you need to wear them when driving.
