What you should know

When you apply for a certificate, you need to:

  • have the treating medical practitioner or midwife sign the application form
  • choose a design
  • decide what information you provide for the certificate.

Who can use this service

You can get an early pregnancy loss certificate if:
Correct.you experienced the loss of your baby—delivered before 20 weeks' gestation and weighed less than 400g in Queensland.
Correct.you are the parent who experienced the loss.
Don’t use this service if:
Incorrect. your baby was born after 20 weeks or weighed more than 400g

Instead, register your child's birth.

Once your application is approved we will post the certificate to you.

Step 2: Get a health professional's signature

Have your treating medical practitioner or midwife sign the declaration section on the form.

Step 3: Submit your application

Submit your application in person at:

Brisbane registry customer service centre
Level 32, 180 Ann Street, Brisbane

Opening hours: Monday to Friday, 8.30am–4.30pm.

Closed public holidays.

Bring with you:

  • completed application form.

Step 4: Receive the certificate

Once your application is approved we will post the certificate to you.

Your application may not be approved if you:

  • don't meet the rules
  • don't include the right documents.

How long it takes

Processing times start when an application with all required documents has been received.

Standard application: up to 10 business days

Urgent application: not available

Allow extra time for public holidays and postage.
