Fill in the access to cultural recognition order records application form
Use this application form if you are applying for a certificate and/or information related to a cultural recognition order.
You don't need authorisation to buy an official birth certificate.
Step 1—Contact the Office of the Commissioner
- Get an authorisation letter from the Commissioner (Meriba Omasker Kaziw Kazipa).
Step 2—Complete this part online
- Start by answering the question below—it should take you less than 15 minutes.
- Download and print the filled-in application form.
Step 3—Complete on paper and submit
- Sign the section about who is applying.
- Have your proof of ID documents and authorisation letter from the Commissioner (Meriba Omasker Kaziw Kazipa) certified by a Justice of the Peace or another authorised person.
- Post the application form along with proof of ID and authorisation from the Office of the Commissioner to Registry of Births, Deaths and Marriages, PO BOX 15188, CITY EAST QLD 4002.
