Proof of identity documents
Proof of identity (ID) documents acceptable for most births, deaths and relationship services.
Most services will accept 3 forms of current ID from either:
- 1 from each category
- 2 from community and 1 from home address evidence.
All forms of ID must be either:
- current – the document has an expiry date and has not expired
- recent – the document does not have an expiry date and was issued no more than 6 months ago.
Proof of name changes will need an additional form of ID for evidence.
ID categories
- Australian photo driver licence
- Australian passport
- Overseas passport
- Adult proof of age card
- Medicare card
- Concession or health care card
- Student ID
- School or other education report from the last 12 months
- Salary advice or payslip
- Private health provider ID card
- Defence force or police service identification card
- Australian firearms licence
- Document of identity issued by the passport office
- Naturalisation, citizenship or immigration certificate
- Birth certificate
- Security guard or crowd control licence
- Government employee photo
- Blue card
- Utility account – such as gas or electricity
- Rent or lease agreement
- Rates notice
- Vehicle registration or drivers licence renewal notice from the last 12 months
- Official letter from other government services (not us)
- Electoral enrolment document
- Insurance policy notice from the last 12 months
Evidence must display your name and current home address details. Only provide the page showing these details.
- Change of name certificate
- Marriage certificate
- Proof of divorce
- Family deed poll document
If your ID documents are not in English, you will need to have them translated.
