Proof of identity documents

Proof of identity (ID) documents acceptable for most births, deaths and relationship services.

Most services will accept 3 forms of current ID from either:

  • 1 from each category
  • 2 from community and 1 from home address evidence.

All forms of ID must be either:

  • current – the document has an expiry date and has not expired
  • recent – the document does not have an expiry date and was issued no more than 6 months ago.

Proof of name changes will need an additional form of ID for evidence.
