You can request to access your prison health record by applying to Queensland Health.

You can use this service to request to access your prison health record.

You must submit your completed application form and copies of your ID documents by email or post.

What you should know

Your prison health record is a paper or electronic record created when you enter prison.

Each time you see your prisoner health team, they update your record.

When you leave prison, we’ll archive your paper record. The West Moreton HHS Central Archives Team stores prison health paper records.

Who can use this service

You can request access to a prison health record if you are the:

  • prisoner
  • prisoner health team.

How to request access to your prison health record

Choose how to submit your application and follow the steps.

Step 1: Complete the form

Download and complete the application form electronically or by hand.

Step 2: Gather supporting documents

Proof of identity

You will need to provide either 1 form of identification from Category A or 2 forms of identification from Category B.

Instructions are found on the application form.

Step 3: Email your application

Email your completed application to the correct correctional centre. Email addresses can be found on the application form.

Step 4: Receive the outcome

You’ll be notified of an outcome once your application is received.
