Medical practitioners can learn how to submit medical cause of death certificates and other forms online.

In short
Medical practitioners can apply to use the Service Provider Portal. This lets you submit:
- Form 9 – medical cause of death certificate
- Form 9A – perinatal supplement
- Form 1A Section A – for potential reportable deaths.
The portal fills in shared details from the Form 9, so you do not need to enter the same information more than once.
What you should know
To sign up and use the Service Provider Portal:
- medical practitioners working for Queensland Health – use your Queensland Health email address and log in with Microsoft 365.
- medical practitioners who do not work for Queensland Health – use your Queensland Digital Identity.
Before we give you access, we will check that you are registered as a medical practitioner with the Australian Health Practitioner Regulation Agency (AHPRA).
What you can do online
Approved service providers can:
- create and submit medical cause of death certificate records
- create and submit perinatal supplements as part of the same record
- create and submit Form 1A Section A as part of the same record
- recall and amend records at any time, even after we accept them.

You can recall and amend records that you have submitted any time. It does not matter if we have accepted it.
If you work at a hospital and they have registered for the email notification service, they will receive an email with the updated certificate.
