        If the sex shown on your current ID is different to the sex on your birth certificate, you must provide evidence showing how you updated your sex on your ID—for example a recognised details certificate from another state or territory.
      2. The sex you identify as
        If your application is approved, this sex descriptor will be your sex under Queensland law and will be shown on your birth certificate. You will need to update your details with every organisation you have a personal account with.

        You must provide a descriptor of the sex that you both identify as and want to live as. Other genders are allowed, such as agender, genderqueer, and non-binary.

      Change of name Do you also want to change your first names? *
      You can choose to change your first and middle names to better express your identity. If you want to change your family name, you need to complete a change of name application and pay the additional fee. Do this after your change of record of sex application is finalised.

      If your application is approved, these names will be your new legal first names and will be shown on your birth certificate. You will need to update your details with every organisation you have a personal account with.

      If your application is approved, you will need a birth certificate to update your identification. Your current one will no longer be able to be used for identification purposes.
      We do not notify you that it has been registered.

      To have everything you need to update your identity in the community, we recommend that you choose the linking birth certificate package.

      You can order additional official certificates:
      • •    Linking birth certificate—shows your new sex and name on the front and previous details on the back (use to update your ID)
        •    Birth certificate (with sex)—shows your new sex and name and does not show any previous details (use after your ID is updated)
        •    Birth certificate (without sex)—shows your new first name (if any) and does not show any previous details (cannot be used to prove your sex)

    You have changed the type of post. We value identity protection, so we recommend that you choose registered post to ensure the delivery of your certificate can be tracked and delivered to you securely within Australia.

    Important update

    Australia Post are suspending their express post guarantee of next-day delivery. Express post is still available and treated as a priority, but may not be delivered the next business day every time. Normal application processing timeframes still apply.

    We value identity protection and you have chosen a more secure delivery outside Australia. However, be aware that international registered mail can’t be tracked, and signing is not available in every country.

    Remember

    You will need to submit your proof of ID, supporting documents and signed application along with payment (if not paying online) to us either:

    • •    in person, by requesting an appointment at the Brisbane registry customer service centre.

Don't use this form

You need to complete a different form if you were born or adopted outside of Queensland and have been living in Queensland for at least 12 months.

Otherwise, see a list of interstate registry offices—each will have different rules for changing your record of sex.
