Correct a certificate
You can request a review to correct a certificate. Apply to the Registry of Births, Deaths and Marriages and paying a fee (if required).

What you should know
For most corrections and updates you will need to provide supporting documents and pay fees for the correction and a replacement certificate.

Correcting a certificate
To correct a spelling mistake or incorrect information you need to read the corrections policy.
Updating a record
In some cases, you can update a record with information not previously allowed at the time the life event was registered, such as adding older half-siblings to a birth record. Read the updating records policy.
If you are adding or updating information about a de facto partner on a death certificate, read the de facto partners policy.

How to request a review of a certificate
Step 1: Fill in the application form
Online form
Complete the online form.
Step 2: Receive email confirmation
You will receive an email confirmation from the Brisbane registry within 15 days. We may ask you for more information if needed.
Step 3: Provide more information
You may be asked to provide more information to support your application. Such as:
- the change to the date of birth on a certificate
- an original letter from the hospital where the birth occurred
- the spelling of a parent's name on a birth certificate
- information about the parent's Queensland birth certificate showing the correct name
- the marriage details on a birth or death certificate
- information to help find the Queensland marriage certificate showing the correct details
- the adding or updating of occupation
- supporting documents from the time the life event happened.
Step 4: Pay the fee
You may be required to pay a fee when we contact you.
Step 5: Receive your certificate
Once your application is approved, the certificate will be posted to you.
Your application may not be approved if:
- you don’t meet the rules
- have not included the right documents.
You will not receive a refund if:
- your application is refused
- you cancel the application after we have reviewed it.

Standard request: up to 20 business days
Urgent request: not available
How much it costs
You may be asked to pay a fee.
- Correction fee: $22.65
- Replacement certificate: $56.20
Regular postage is free.
You can select from delivery types:
- Registered post (recommended): $8.10
- Express post: $8.55
- International registered mail: $18.55
