How to apply to withdraw an application to end a civil partnership

Step 1: Gather your documents
Proof of identity documents
You need to submit 3 forms of proof of identification (ID).

Translating documents
If you need to provide proof of ID or supporting documents and they are not in English, you need to get them translated.
Step 2: Get your documents certified
You must get copies of your proof of ID documents certified.

Step 3: Choose the application form
Online form
Complete the online form and print.
PDF form
Download, print and complete the form by hand.
Step 4: Get your signatures witnessed
An adult (18 or older) must witness your signature(s) on the application form.

Include your:
- completed application form
- certified copies of ID and supporting documents.
Step 6: Receive confirmation
Once your application is approved, you will be notified the application to end a civil partnership has been withdrawn.
Your application may not be approved if you:
- do not meet the rules
- have not included the right documents.
You can submit your application at any Queensland Magistrates Court or QGAP office (except at the Brisbane Magistrates Court).
