You can apply for a long service award if you have served as a Justice of the Peace (JP) or Commissioner for Declarations (Cdec) in Queensland for 25, 40, 50, 60 or 75 years

What you should know
After your application is approved, your state Member of Parliament (MP) will nominate you for the award.
They will present the award to you.
Who can use this service
You can apply for a JP or Cdec long service award if:
- you have continuously served as a JP or Cdec in Queensland for 25, 40, 50, 60 or 75 years.
Don't use this service if:
- you have not served in Queensland for at least 25 years
Instead, wait until you have served for 25 years.
How to apply for a JP or Cdec long service award
Follow these steps to apply for a JP or Cdec long service award.
Step 1: Complete the form
You must provide the date (or approximate date — month/year) you were appointed as a JP or Cdec.
Step 2: Submit the form
You must confirm that you:
- agree to your application being sent to your local Member of Parliament for nomination
- are the person listed in the form
- have read and agree to the privacy statement.
Submit the form. It will be automatically sent to your state MP and they will review your application.
Step 3: Confirmation of nomination
Your MP will review your application, and if approved, they will confirm your nomination.
Your award certificate will be prepared and signed.
Step 4: Presentation of award
Your MP will contact you to arrange a time to present you with your award.

It can take up to 4 weeks to process your application and deliver your award.
